Log into the Patient Health Portal, by going to:

https://studenthealth.temple.edu/

and click on the Patient Health Portal Login link:

Then enter your Temple Credentials (same credentials used to log into TU Portal):

Complete your MFA Authentication:

Click on Uploads

Choose the Immunization Record upload and select the file with your Immunization information in it (If you are a Clinical Student, you will use: HSC and CPH Immunization, Titer And-Or Physical Record), then click the Blue Upload button.   ***A Waiver for Immunizations will be available under the FORMS section.

Please only click Upload once.  Hitting it more than once will upload multiple copies of your file.

PLEASE NOTE: After you upload a copy of your immunization(s) record(s), they will be reviewed by a Student Health Service staff member. If anything further is needed, you will contacted via email.
