Applying for Medicaid in Pennsylvania

Requirements:

1. You have lived in Pennsylvania for 45 days
2. Your monthly income is ~$1500 or less

Please make sure you gather the information needed.  Below is all of the information that is required to apply for Medicaid.  Please take pictures of all the information as you will have to upload the information from your phone or computer to the website.

Documents needed to apply for Medicaid

1. IDENTIFICATION/STATE ID
2. VISA- IF YOU ARE NOT A UNITED STATES CITIZEN
3. VERIFICATION OF INCOME – 1 MONTH OF PAYSTUBS
4. IF NO INCOME - LETTER OF SUPPORT from parent or guardian
5. VERIFICATION OF WHERE YOU LIVE – A LEASE OR UTILITY BILL IN YOUR NAME
6. IF YOU DO NOT HAVE A LEASE OR UTILITY BILL IN YOUR NAME, YOU NEED A LETTER FROM THE PERSON WHO LIVES WITH VERIFYING YOU LIVE THERE.
7. TWO MOST RECENT BANK STATEMENTS IF APPLICABLE.
