In order to be compliant with the Philadelphia Covid-10 mandate, patients will need to upload their Covid-19 vaccination record in Patient Health Portal. This is a 2-step process. Patients enter their Covid-19 vaccination dates on the Immunization tab, then complete the process by Uploading their Covid-19 Vaccination record.
